That was a HUGELY helpful post, and I hope others that have are trying to make ragdolls (and suck at matrix math) get a chance to read it.

My problem definitely had to do with switching spaces, and the way in which I was calculating the initial offset. I still have some tweaks to make in initial alignment and rigid-body creation, but it WORKS!
